How to Request a Home Office Number

For providers claiming cost on their cost report reports that are allocated from a home office or chain organization, a Home Office Cost Statement must be filed annually.

To request a Home Office number, please prepare a letter with the following information:

  • Home Office name 
  • Physical location of the Home Office to include street, city and ZIP Code
  • Correspondence address of the Home Office
  • Date the Home Office was established
  • Fiscal year-end of the Home Office
  • Contact for the Home Office to include name, title, phone number and email 
  • A prepared Schedule A from the Cost Report listing the components 

Please submit the letter request and Schedule A to STAR@palmettogba.com; which will then be forwarded to the Home Office Enumeration Team at FSS (Federal Specialized Services) for a home office number assignment.
